Teleporting after portal creation is not smooth and reliable

Tried this on 0.4.14, you create a portal, stand in it, you see grey screen now, and you are back again at your portal (not in nether), you try to go in it again, but yet again the process is sloppy and you can end up in nether or back at initial portal.
Process is sloppy and confusing, this needs clean up.

While experimenting this has happened to me, if you're not fast enough exiting the portal the ABM on the portal nodes can teleport you back and forth between the two portals. I've been tuning the ABM to work slower.

We can either teleport the player to the side of the portal (and not into the new portal) or add some sort of delay / timeout.

A timeout table shouldn't be too difficult.

I wonder why there is no onContact function, that runs if player gets in contact with block or steps on it...
