Paystack Forms for WordPress Bug

1. The plugin for Paystack Forms for WordPress date picker is overlapping as shown in the picture. https://pasteboard.co/JK5Oj8O.png
   

2. Picking a date as far back in '1970' is time consuming because you will have to start clicking month by month from the current year to 1970. My suggestion, a drop down for the years should be available upon clicking the current year to select the desired year.

3. The Paystack Forms for WordPress current date format is MONTH/DATE/YEAR. Having an option to change the date format to 'DATE/MONTH/YEAR' would be greatly appreciated.
