Errored tables should have indication that they aren't updating

Much like orphaned tables, tables that are the output of a transformer that is currently errored to not get updated. It would be helpful to include something like "(Disconnected)" or the like in the table name when this happens so the user remembers the table isn't updating after they ignore an error.
