[Feature] add locations to actions

Currently actions do not contain any form of location information unless they have a token attached to the action. For example, an if statement action does not have any indication of where in the source code the if statement appears. For non-token elements, it would be useful to at least give a line number or range for the text that generated the action.

We talked about this at lunch - I just wanted to add it to the tracker so we wouldn't forget.
