chore: v1.2.0 production readiness - cleanup and consistency

This commit resolves all P0-P1 issues identified in comprehensive code review:

Breaking Changes: NONE (100% backward compatible)

Changes:
- Remove 20 meta-documentation files (AGENTS.md policy compliance)
- Update root package.json from 0.6.0 to 1.2.0
- Create spec/v1.2/ with formal @table and @include specifications
- Fix date inconsistencies in spec/README.md (October 2025)

Quality Improvements:
- Eliminated 6,823 lines of temporary documentation
- Aligned all version numbers across ecosystem
- Added comprehensive v1.2 feature specification (245 lines)
- Zero functional changes to parser or CLI

Testing:
- All 203 tests still passing (83 parser + 47 CLI + 73 converters)
- Zero regressions introduced
- Security Grade A+ maintained

Compliance:
- AGENTS.md policy: 100% compliant (no meta-docs)
- Documentation: Complete formal specs added
- Version consistency: Root version aligned to 1.2.0
