Problem with form with LOINC 46611-0

Hello,

I noticed a problem when rendering a form with LOINC 46611-0. You can observe my issue if you open the jsfiddle link below.

If you run the jsFiddle, the form will be rendered. Scroll down to the bottom of the form and you will notice that the field "Date form completed" is expecting a number instead of a date (see image 1).

It should render like in the Image 2 below, which by the way it is the exact same form that I pulled from "Search LOINC Panels" from the lhcform tools site (https://lhcforms.nlm.nih.gov/lhcforms).

It is a data issue from LOINC. See https://loinc.org/46611-0/ and https://loinc.org/30947-6 The "Date form completed" is defined as a quantity type.
