Steps to Reproduce the Issue
It seems that two fields were deleted from at least two DREFs Ops Updates when user saves. The cases are:
Venezuela Complex Emergency Ops Update
Dominican Republic Hurricane Melissa Ops Update
In the case of Venezuela I was able to check the admin site for the history and saw weird behaviour where fields in one of the versions of the document were replaced by “none”, meaning deleting the content.
I have not replicated this bug myself.
Expected Behavior
content should not be deleted upon saving
Actual Behavior
Content in the ops update for the fields “summary of changes” and “any gaps in the assessment report” is being deleted or replace by “none”

Priority
High (Major functionality is broken)
Additional Context (Optional)
I have not replicated this and in the case of Dominican Republic the users had already added the content back and I could not find the version when content was fully deleted by the system, Venezuela case is better to explore. The versions are compared are:
- Jan. 26, 2026, 12:30 a.m. - Santiago Rodriguez
- Jan. 27, 2026, 4:46 p.m. - Carlos Ortega.
In the screenshot you can see the two fields being deleted and replaced by None, you can see other fields were also sort of replaced by “none” such as the date of event and the assessment report.
