Create upload form for musical work

Use files from 2019 repo, update form according to newer specs:

• User can add multiple contributions + contributors for one musical work
• User can pick the ordering of their section (i.e. the first section defaults to Kyrie but the user can specify)
• Genre (style & type) and location autofills from database (but new styles, types, and locations may still be inputted)
• General css improvements, particularly making spacing regular
• Eventually will have autocomplete form VIAF and Wikidata (this is marked in another issue)
