docs: Rewrite README for clarity and completeness

- Lead with what the project does and who it's for
- Show the phone-to-machine flow visually
- Add "What you see in Telegram" examples upfront
- Reorganize setup as numbered steps with gotchas inline
- Add complete config reference table with all fields
- Security comparison table (TypeScript vs Rust)
- Module table with line counts
- Troubleshooting as table format
- Full hook installation example including SessionStart/End
- Remove duplicate sections, tighten prose throughout
