Allow user to set DOB on preference page

Current Implementation and Limitations

Currently the only time the DOB is set is when an admin creates the user account. There needs to be a way for the user to do this since the admin may not know this at the time of account creation.

Add a DOB field on the prefernce page:

The page is pretty useless now, so it gives function to an empty page!
