Add search bar to homepage #30

Overview
Add the search bar to the homepage. We have not selected a autocomplete library nor a place lookup, but please do your best to add the functionality for the drop downs shown in the wireframes below. This does not include adding search functionality yet (though if you have a solution in mind for that please go ahead)

Bootstrap doesn't seem to ave a straightforward Search component, but the Input Group may have something we could use. I recommend looking through the Bootstrap documentation to find something useable.
