As a case manager, I want clients to complete an online assessment, so that I can provide them with recommendations for targeted interventions based on their results.
Acceptance criteria:
The form should be completely filled out before clients meet with social worker
When a client fills out their form, they are given a percentage baseline
The social worker has access to the information the client output
The client has access to their own form
The client receives confirmation of receipt of their form to their email address
The case manager can make edits to the form if needed
The information is confidential between client and case manager
