Simplify List and Folderless List into a single table/schema/stream #97

Just like what was done with Task I believe that it makes sense to simplify List in the same manner. By putting it into a single table/schema/stream (not sure of the correct term here) it'll make querying the data from a single table much easier and will handle duplicated records from when List may be moved from outside a folder or from when a Folderless List is moved into a folder. Our team confirmed that both of these errors were happening while running incremental changes.
