feat(refresh): show session progress

Summary

• Add shared CLI progress rendering for spinner steps, throttled progress bars, and non-interactive-safe output.
• Show refresh progress for project sync, indexing, and overall project completion.
• Wire sync and index engines to emit session-count progress while preserving existing no-progress APIs.
• Add tests for refresh/share progress rendering and throttling behavior.
